Memory is an Attribute in Divinity Original Sin 2. All characters begin with a basic 10 points in each attribute. Characters can be customized with 3 additional attribute points at the start and acquire 2 more points per level. A total of 30 points can be added to an attribute for a maximum of 40 points. This maximum can be further be augmented through equipment. The Lone Wolf talent allows players to double their investment in attributes and combat skills.
Information about Memory
Memory increases the amount of Skills you can equip (memorize) and use at one time. More powerful skills require a higher level of memory and while you can learn every skill in the game, only a limited amount of them can be active at one time. Outside of combat, characters can organize their active skills to prepare for their next encounter.
Notes for Memory
- Each point in Memory grants 1 memory slot.
- Characters gain an additional Memory slot every 2 levels. (2,4,6,8,10,12 etc.)
- Individual skills can cost anywhere from 0 to 3 Memory slots.

For Clarity: Characters gain an additional Memory slot every [2 memory attribute levels, or every] 2 [character] levels (2,4,6,8,10,12 etc.). Slots = (Character Level)/2 + (Memory Attribute Level) - 7

aside from whithermoor’s belt are the here any gear that gives plus to memory (same with polymorph)

Confirming that you do gain 1 slot every two levels. I'm currently at level 35 with 20 slots with a base 10 memory.
The Wizard Hat Mushroom adds to memory (+1) temporalily (2 turns) Been tryin to find a potion recipe for it, but so far no luck
